Ramps are built to the Americans With Disabilities Act (ADA) standards which require a 12:1 slope.  This means that if your deck is 36 inches off of the ground you must have a 36 foot long ramp which is 4 feet wide.  It also requires a 6 foot by 6 foot landing area at the top of the ramp.  Prices vary as to the length of the ramp and type of material used.  The ramp pictured is a 36 foot ramp with a 6 foot by 6 foot landing at the door.  Please note the tree.  Special considerations had to be made for this obstacle.
